Taylor Dental Associates is pleased to welcome you as a new patient. This downloadable packet of forms must be completed prior to your first visit. Each form contains information which helps us to better serve you, or provides you with information we feel you will want to have.

Please take a few minutes before your appointment to print the forms and fill them out completely. Bringing the competed forms to your first appointment will save time on that day. (The following forms are PDFs and can be viewed and printed using Adobe Reader.)
